Why good-looking corrective actions still fail?

Most teams do not struggle to create corrective actions.

After an incident, failure, defect, or recurring problem, the action list usually fills up quickly.

  • Update the procedure.
  • Retrain the team.
  • Inspect more often.
  • Add a checklist.
  • Review the risk assessment.
  • Talk to the supplier.
  • Replace the component.

On paper, it can look like progress.

The investigation has been completed. The actions have owners. Due dates have been assigned. The report can move toward closure.

But a few weeks or months later, the same issue appears again.

Maybe not in exactly the same way, but close enough for people to recognise the pattern.

That is the frustrating part.

The team did the work. The actions were closed. The report looked complete.

So why did the problem come back?

One reason is that corrective actions are often chosen before the problem is properly understood.

When the causes are shallow, the actions usually become shallow too.

If the cause is written as “operator did not follow procedure,” the action becomes “retrain the operator.”

If the cause is written as “equipment failed,” the action becomes “replace the equipment.”

If the cause is written as “inspection missed the issue,” the action becomes “increase inspection frequency.”

Sometimes those actions are useful. But they may only treat the point where the problem became visible.

They may not address the conditions that allowed the failure to happen in the first place.

This is where Root Cause Analysis needs to be more than a report.

A useful RCA should help the team connect actions back to specific causes.

Not just “what can we do?”

But:

  • What cause are we trying to control?
  • Will this action reduce the chance of recurrence?
  • Is this action practical in the real work environment?
  • Are we relying on people to remember, notice, or try harder?
  • Have we reduced the risk, or just added another task?

These questions matter because busy teams often accept actions that sound reasonable but do not change the system.

More training may not fix a confusing process.

More inspections may not fix poor design.

More reminders may not fix competing priorities.

A new checklist may not fix missing information, poor access, unclear ownership, or weak handover.

That is why understanding must come before fixing.
