Let's break down the RCA process into 5 simple steps...

1. Gather and Manage Data/Evidence

2. Create the Problem Statement

A well-defined problem is halfway to being solved. Frame your problem statement in a way that answers three key questions: What happened? When did it happen? Where did it happen? Keep it factual and concise.
 

3. Analyze Cause and Effect

This is where curiosity shines. Use a cause-and-effect chart to map out the sequence of events leading to the problem. Look for contributing factors, not just the obvious ones. Asking “Why?” 5 times (but with a structured approach) uncovers deeper causes.
 

4. Generate Solutions

Once you understand the root causes, brainstorm solutions that directly address them. Aim for actions that are specific, measurable, and sustainable. Quick fixes might be tempting, but lasting solutions save more time and money in the long run.
 

5. Produce the Final Report

Document your findings and corrective actions. A clear, concise report helps communicate the solution to stakeholders and ensures accountability. It also serves as a valuable reference to prevent future issues.
